Advise

First, you will be asked to complete a 15-20 minute health survey. You’ll give ContinuousHealth information about yourself, your preferences for health insurance companies, and your health history. You’re disclosing this information only to us. Your employer will never have access to this confidential information. By law, ContinuousHealth can only use this information to make a recommendation about a health insurance plan for you, and to apply to the insurance company on your behalf. Your personal information is protected and safe.

Acquire

Once you have provided your information to us in the health survey, we will study your needs, research various insurance companies to determine the best options for you personally, then recommend 3 – 5 plans we believe will be right for you. You will study our recommendations, ask us any questions you may have about the recommendations , then choose a plan that fits your needs and the needs of your family. If you don’t see something that exactly fits your combination of budget, doctor choice and covered services, ContinuousHealth will do more research and make other recommendations. At the end of the process, you’ll have the ability to choose the plan that is right for you.

Assist

The next step is application.. Now that you have chosen the plan that is right for you, ContinuousHealth assumes the responsibility of obtaining coverage for you. We take all the information you provided and complete the paperwork required by the insurance companies. We ask you to review the application to be sure everything is accurate. We submit it to the insurance company on your behalf. We follow up with the insurance company until your coverage is approved, and until your ID card is in your hands. You begin using your health insurance plan the way you used it in the past except now, it is the plan that you chose. It is one that is right for you.
